1. Attorneys typically initiate referrals following a court order to secure reunification services. We will screen the case to make sure it is a good fit. If you are pro se, unrepresented by an attorney, we may decline the case. We strongly encourage you to work with an attorney through the reunification process. If we accept the case, your attorney will provide us with the following contact information, which we will forward to our assistant, Dendree Knott: Name, Phone Number, Address, Date of Birth, Email, Relationship to Child/ren. She will then send you instructions to complete Intake paperwork.

  2. Your attorney or you will provide a copy of the Court Order appointing Michael Alter, LCSW, Reunification Works, Inc., or a provider on my team, as the reunification specialist / expert / therapist. We will review the Order to make sure it meets criteria for effective reunification. We will discuss any needed changes or modifications to the court order with the attorneys involved. We usually provide a Form of Order Template, which attorneys may use to customize the Order to the clinical needs of the case. We cannot begin services until we have an acceptable court order in place.

  3. Dendree Knott will contact you with instructions to create a client profile in the CarePaths program so case activity, charges, and records can be tracked. You will have access to the portal to complete Intake Forms, and be able to track invoices and activity. If you have questions, contact Dendree at 971-318-0921 or dendreeapms@gmail.com. Tell her this is a reunification case.

  4.  Complete all Intake Forms in the CarePaths electronic health record (EHR) system, where they will be uploaded automatically in your portal. If you have any problems with the CarePaths system, contact Dendree for assistance.

  5. Please sign ROIs (Releases of Information) for our providers to consult with every therapist you and your child or children are seeing, or have seen in the last two years.

  6. Post your percentage of the $3,500 retainer, per allocation fees in the the Court Order. This may be done by check, cash, money order, cashier's check, or authorized ACH payment. Please send payment to Michael Alter, Reunification Works, P.O Box 12553, Portland, OR 97212. Charges are processed weekly. You must replenish your retainer when your balance is at $250 or below.

  7. Your attorney or you will provide contact information (name, email and phone number) for others who may be involved. This may include a parent, step-parent, guardian (when applicable), child/ren (when appropriate), attorney, custody evaluator, parenting coordinator, parenting supervisors, mental health therapists, and relatives or significant others. Some of this information may be completed on the Demographic Form in your intake paperwork. We also request relevant reports and evaluations, such as the Dissolution of Marriage Agreement, General and Supplemental Judgements, and parenting time agreements.

  8. After all requisite steps are completed by both parties, we will begin scheduling appointments pending our availability.
